Before the Ketamine Intake Appointment
Once your intake is booked, we will send you a set of required Tebra intake forms that must be completed and submitted at least 48 hours before your scheduled session. This allows your provider enough time to review your background and prepare for a meaningful, thorough evaluation.
These forms include:
- Basic patient information & demographics.
- Financial information (insurance is not accepted for ketamine therapy).
- Medical information (allergies, current medications, medical history, mental health screening questionnaires, etc.).
- Consent and policy forms (telehealth consent, privacy policies, practice policies, etc.).
- Ketamine-specific informed consent (sitter requirement, contraindications, safety guidelines, BP monitoring requirement, etc.).
- Information sheet (start-to-finish process of ketamine therapy, rules, etc.).
- Preparation resources (calming music playlist, required and recommended items).
Completing these forms ahead of time ensures a smooth and efficient visit.
During the Ketamine Intake Appointment
During your session, your licensed provider will:
- Complete an in-depth mental health and medical assessment.
- Review any safety considerations and potential medication interactions.
- Discuss treatment expectations and dosing.
- Explain what to expect during and after sessions.
- Explain vitals requirements (blood pressure and heart rate).
- Review your responsibilities during treatment, including vitals monitoring, designating a sitter, and therapy requirements.
Blood Pressure Monitoring
You must have an FDA-approved blood pressure cuff at home. Your provider will explain how and when to check your vitals before each dose. Weekly blood pressure reporting begins once you start treatment.
Sitter Requirement
A responsible adult must be present with you for the entire duration of each dosing session. This is required for your safety.
Therapy Requirement
Patients participating in ketamine therapy must see a therapist regularly. This helps support emotional integration, safe processing, and long-term progress.

What are the possible side effects of ketamine therapy?
